Pyometra

Saurabh Saxena now in 3 consecutive cases of open pyometra ,where cbc showed neutrophilia(high 80s and early 90s),after 3 days of drips and antibiotic and DNC ,when shifted to oral antibiotics,the patient suddenly dies??is it just a coincidence or am i missing sumthg??

    • Amardeep Singh Doc what is TLC, i always advise the owner for OH..... what does the group sugests ... let me know . high TLC hIGH nEUTRO , uterine discharge is almost confirmatory for PYM.

    • Saurabh Saxena hi doc ,thnx .i advised for OH only but was trying to lower the infection load by going for drips and antibiotics.it was responding very well and started moving ard only wen i gave a break of few days by shifting to oral antibiotics it suddenly died.wat ud be the reason .we had d x ray and cbc all confirmatory of pyometra..

    • Amardeep Singh Did u get biochem - OT/PT, BUN & Ser creat done or not could have been renal / hepatic damage too. what was the Hb % in cbc. at times low Hb also becomes fatal

    • Saurabh Saxena hb was 11 and ot/pt bun /cre were all normal .dont hv exact values as report given to owner.

    • Joseph Cyrus In certain cases of gram negative bacterial infections antibiotics are contraindicated as they may lead to endotoxic shock!

    • Joseph Cyrus not in all cases of bacterial infection may u use antibiotics. though in practise we do it. it is not the case, some gram negative bacteria release endotoxins when a bacteriocidal antibiotic is used especiallly in an overwhelming infection like pyometra. Thus it may be wise to remove the uterus and then start on antibiotics. It is there in most gynaecology text books
