Post Dog bite emphysema

DrKumar Ravi Dr vets a four day old dog bite(1 inche) case in a 6 yrs old dog comes to me, there is a painless swelling spreads to whole of caudal body with bubble crackling sound every where white there no such great dischagre . I have started augmentin 625 mg, serratopeptidase, meterogyl0to combat anarobic bacterial inf. As i feel for 5 days. There is no way to dress wound deeply. I think of opening the wound but hoW MUCH I OPEN IT SPREADS TO A VERY LARGE PART. Give suggestion to cure

    Gautam Unny dr kumar i have always advised you to open any dbw at least n inch on day 1 of the bite, it increases the drainage and prevents such incidents

    @dr swarup as per my knowlege gangrene occurs on extremities n in four days its not possible

    Flushing the wound with H2O2 or Potassium Permangnate solution.

    Swarupmay Majumdar why not possible sir? Gangrene is nothing but a gas filled wound!! simplified. THis gas may be produced by the anerobics, seen many cases getting benefitted with Cephalexin alone.

    Rajshankar Tony Sarma Clostridia; also subcutaneous emphysema is a sign of (esp extrathoracic) airway rupture - tracheal possible with dog fights. Usually get other respiratory signs though.

    ‎...... sometimes the area can get spread very far and look much worse than what it is! the idea is to leave it open. do you know how to place a drain in the wound? if you need advice to put a simple one in you can message me..... if you do not have a rubber type of drain even a finger of a surgery glove cut off will work.... i say use something broad spectrum or perhaps enrofloxacin.... and get a good long drain placement....position it to work with gravity. You can flush the wound vigorously once a day with sterile saline even just to get the dead tissue away... the body does a lot of healing on it's own if it is allowed to....
